Usually the correct choice here is the Act 1 boss fight, and then to skip the rare relic when it is offered.

Mind Bloom: Do NOT take the option to upgrade all your cards in exchange for no healing.If Neow isn’t offering the boss relic trade, you have to take the max HP. Neow: Do NOT accidentally take “Enemies in your next three combats have 1 HP.” It will give you a relic.But relics like Cursed Key, Snecko Eye, or Pandora’s Box can provide a huge advantage. Black Star and Calling Bell in particular are probably worth an immediate restart. Neow: The boss relic swap is a very strong choice, as it tends to make your only relic stronger.First, and most importantly, there are a few events that you must be extra wary of: Still, there are many points to keep in mind that may make the achievement easier to obtain. You must simply play well and try until you succeed. There is also no trick or shortcut I can give for this achievement. Not only is the lack of relics (including boss relics) crippling to the player, but many people struggle with turning off the muscle memory to pick up every relic they see. For many players, it is the hardest of the four challenge runs, and it is the least achieved of the challenge runs on Steam.
